Andy is probably one of the most common known, how'd you leave that one out? :D
If you have a good character, Baal can drop any item in the game in Hell difficulty, and I think NM baal is the most probable to drop HoZ, or was it NM Mephy?
Countess is good for Rune Runs, and Lower Kurast Super Chests.

so, it's baal (nm). do note that the first diablo on the table refers to the dclone. so such drop is considered inapplicable.

Agreed. Go with ancient tunnels and Pit in hell mode if you don't have character that can teleport to bosses. Ancient tunnels are nice to run with cold sorceress, or meteorber as only cold immune can be random unique spawn - you'll need merc to handle them.
